Participants enrolled in the housing program may be eligible to receive up to $15,000 per the 2026 year in Housing Transition Assistance to support costs related to securing and maintaining stable housing, subject to funding availability. Funding is renewed annually and does not roll over if unused.
Eligible uses of Housing Transition Assistance may include, but are not limited to:
Identification and documentation costs
Move-in and relocation expenses
Rental or storage arrears that present a barrier to securing housing
Essential household furnishings and basic household needs
Additional eligible expenses may be considered on a case-by-case basis. All requests for Transition Assistance are reviewed and evaluated by program leadership and submitted for approval to Wellpoint, the program’s administering entity.
Funds are never issued directly to participants. All approved payments are made directly to vendors, landlords, or service providers. Move-in related assistance must be applied toward a new housing placement at the time of the participant’s move.
